How to make medical decisions by leveraging 3 tiers of expertise

When facing serious medical decisions without foundational knowledge, most people default to trusting their doctor or seeking a second opinion. However, this approach may prove insufficient, especially for complex conditions. My own experience with Non-Hodgkin's Lymphoma—requiring choice between stem cell transplant or CAR-T therapy—prompted me to develop a structured decision-making framework.

Three Core Steps

1. Adopt a CEO Mindset

Rather than passive acceptance, position yourself as the decision-maker. "However uncomfortable this may be—you want to consider yourself the CEO of your healthcare." This mental shift transfers agency from medical professionals to the patient, recognizing that you ultimately bear responsibility for treatment choices.

2. Visualize Your Decision

Document the problem through pros/cons lists or probability trees. This accomplishes two goals: it clarifies your own thinking, and it creates an artifact to share when seeking guidance from others.

3. Leverage the Magic Pyramid

A three-tiered expertise model provides comprehensive perspectives:

  • Specialist Doctors (Top): Your primary physician plus additional expert opinions regarding your specific condition and medical history
  • Adjacent Experts (Middle): Researchers, scientists, or professionals understanding the biology and emerging treatments
  • Smart Generalists (Bottom): Business leaders and decision-makers comfortable navigating uncertainty

When consulting generalists, frame discussions as hypothetical scenarios: "Imagine this situation. How would you approach it?"

The Breakthrough

Through multiple conversations with smart generalists, I discovered "only one thing mattered—the speed of learning." This singular variable emerged independently across consultations, transforming confusion into clarity and enabling informed decision-making.
